Business Problems:
Goal: Improved integration of clinical, administrative, business and marketing activities. Improved patient flow, quality of delivery of care, inventory management, accounts receivable, claims management, interaction with referring physicians, and internal marketing. Solution: Implemented ifa EMR.The integration of ifa EMR with testing equipment allowed for a more efficient and speedy patient flow through the exam process. The digital interfacing of the equipment to the EMR (see list of equipment utilized in the practice below) eliminated the double entry of data, i.e. with the push of a button, results of all test results were immediately transferred to the patients records and made available for review by the doctor. Results: Since going paperless in March 2002, the practice has realized unprecedented growth in patient and surgical volume.Patient volume growth has increased at the rate of 20 percent a year, resulting in the doubling of patients seen in the last five years. This also yielded revenue increases of more than 100 percent in the last five years (income doubled).The practice continues to grow at the rate of 20 percent a year, six years in a row. EMR allows for higher income per patient encounter. Through the decision support technology, all the necessary testing is performed, interpreted and billed in a timely manner. Case Study: Determined to provide the best eye care possible, Dr. Benjamin has always believed in the importance of having a cutting-edge facility, designed progressively in terms of interior design and organization, and equipped with latest information technology. The practice was incredibly successful from the beginning and the volume of patients increased dramatically. Endless writing seemed to take too much time from the doctor and his team members. Also, such factors as the administrative ease of a well-managed business, medico-legal issues, insurance claim submissions and audits, rapidly changing industry standards, and governmental regulations influenced his decision of EMR implementation. In 1999, Dr. Benjamin began using Electronic Medical Records. He investigated various options. Initially, the MediNotes charting plus system was implemented. Although the system was fairly inexpensive and easily customizable, implementation was not easy. Because the EMR lacked instrument interfaces, double entry was required for test results. Test results such as visual field print-outs required scanning of printed documents into the patient file. After using the system for a thousand patients, Dr. Benjamin began to look for something else. He knew that abandoning EMR was not the solution, that a better system was needed for his increasingly busy practice. The search continued. Some companies had well-developed EMR, but ophthalmology wasn't their specialty. Dr. Benjamin did not want to ever be abandoned by the discontinuation of EMR support. Others were trying to sell EMR as part of office management software that would require new expensive changes. Few companies offered the low-price solution to use EMR via internet in order to eliminate the hardware costs. They even forgot to mention that in the event of contract termination, they would keep all the existing data or sell it for a price higher than new hardware, installation, and training of staff all together.
